What Is a Rule 25 Assessment?
A Rule 25 Assessment is a comprehensive substance use evaluation based on Minnesota Department of Human Services (DHS) guidelines. It is designed to determine whether a person meets the criteria for substance use disorder treatment and to recommend the appropriate level of care.
Our assessments follow the Minnesota Matrix and consider six dimensions of clinical need, including:
- Acute intoxication and withdrawal
- Medical and mental health concerns
- Emotional and behavioral conditions
- Readiness to change
- Risk of relapse or continued use
- Recovery environment and supports
These assessments are often used in legal, school, or personal contexts to access treatment and funding.
Who Needs a Rule 25 Assessment?
You may need a Rule 25 assessment if:
- You're seeking treatment for substance use
- A court, probation officer, or employer has requested one
- You’re exploring insurance- or county-funded treatment options
Cost and Coverage
- Insurance: In many cases, the assessment may be billed to your insurance provider, depending on your plan and medical necessity.
- Out-of-Pocket: If not covered by insurance or if you’re uninsured, a self-pay rate may apply. Please contact us for current rates.
